Need help with Paperwork

This summer I will be filming my first documentary (first film overall) at an air force base and will be interviewing cadets stationed there. My question is what, if any, legal release forms/waivers etc. will I need and where do I get them? Any response apprciated.

Personal appearance release forms, any contract that gives you permission to use their name, image and likeness in the film and for any promotional purpose.

And location release forms. Don't want any problems with showing the base.
