ACTOR AGREEMENT FOR A MINOR

I am not a lawyer but you should be able to modify a standard release. Have both the child AND the parent sign it.
The last line could say:
The parties acknowledge that I am a minor and that my parent/guardian has consented to my appearance in the Work and the terms of this release.

and it's the same as the adult one, but you have a second signature line added that says "parent or legal guardian of _____________"

Maybe if it's just the child. Definitely NOT void if you have child and parent signatures on there. I was told by a California entertainment lawyer in 2007 that you should have BOTH the child AND a parent sign it. Have the contract worded so that the agreement refers to them as "We".
