public domain footage paperwork

I need to send off the paperwork for a documentary, and some of the photos and footage I've used in the documentary were a public domain.
I've never dealt with public doman material, so how should I mention that in the paperwork?
Should I print something out of the source websites?

We want to use public domain or royalty-free music or photographs. What do we do about the Music Release Form or the Materials Release Form?

Have the person who has the rights to the music or materials sign the release form. In the case of royalty-free materials, this is the person who purchased them. In the case of public domain materials, this could be anyone on the team. Please also include documentation that shows your rights to the music or materials, such as a license, a purchase receipt, or a statement by the author. Note: You may only use materials if the license granted to you allows for all forms of distribution.
 
I would think that public domain would be different from royalty free. Royalty free refers to intellectual property that you can purchase a non-exclusive license to where you only pay the one time regardless of number of uses. Public domain is intellectual community property as it were, where you don't have to pay for or ask permission to use it. pdinfo.com is a useful resource.
