I need a blank Personal Release/Waiver Form

I am shooting a documentary, many interviews, but I am asking people to sing as well as talk. I need a release form for them to sign. Does anyone heve one? I found on ein an old post but can't seem to downlaod that one. Thanks - I'm really pressed for time!!!!

A standard release should work fine. Just have it say "my picture, voice, creative input, performance, silhouette, and other reproductions of my physical likeness (as may appear in any still camera photograph, video tape, and/or action picture film)...."
