Vegas plug or other way to do Medussa effect?

If the camera and actor are not moving you could do it with a simple mask using a stone paint job on the actor in photoshop.

You can roto the actor and overlay a stone texture (animated preferably). The stone texture would need to be "crawling" the entire shot because you wouldnt be able to lock it into place on the actor. The actor will "slide" under the texture.

The "pro" option would be to replace the actor with a 3D model.... but I dont think that fits your question.

Presumably the way to do that is with a locked down cam and then use a frame grab? To insure the actor won't move?

What do you mean "slide under the texture"?
 
It would be like projecting an image onto the actor with a projector. If he moves, the pattern would stay the same not move with him.

You could shoot it with everything but the actors moving. Their eyes moving would help sell the effect.

You could try this in After Effects with the puppet tools, but thats going to be a lot of trouble if you dont shoot it correctly.
