Insurance and release forms

Hi all

I've searched the forum, but apologies if I'm missing an existing post...

I'm starting to do paid work - short films promoting local services, work with theatre companies, music videos, etc.

I'm keen to be 'legit' in the most basic sense, which I imagine means holding personal liability cover. Do any UK-based film-makers have advice about this? Is a bare-bones cover all that is needed? What might I expect to pay?

Equipment insurance - do people use this? Have they linked it with other insurance cover and got a good deal?

Finally, to help me gauge when 'personal appearance' release forms might be needed... I'm shooting a dance class next month. Should I expect to need release forms from participants (all of whom will be notified by the teacher)? There seem to be grey areas when I search around for info...
