Help! TV Cue Sheet & Copyrights Return

Hi Folks,

I've just completed my short film and now I really need help with 2 forms...

I am presuming the TV Cue Sheet is used for the music in the film? My composer owns copy-right, but I own the usage. I just don't understand the 'Tape accession number' & 'Part number' etc.

Any advice on filling it out would be appreciated!!!

As for the Copyright Return form, is this just for footage? I own the rights to ALL the footage. It asks for details of owner for stuff created not specifically for the film (but everything was made for it), so do I leave this blank? Do I have to list every single shot?

Again, any help here would be awesome! I need to have these filled in within the next 2 days. I'm based in UK/Ireland if that makes any difference.

The music cue sheet is because of cokmpanies like ASCAP and BMI that will collect royalties for all the music being played during a broadcast or DVD release of the movie. This will all be based on the Music Cue Sheet information for disbursement of royalties.

You can find a more simplified form that doesn't have TAPE ACCESSION NUMBER or PART NUMBER, as I have no clue what those are either.

As for the COPYRIGHT FORM, yes, they are asking for clearances for anything you didn't create, so leave it blank if you didn't use any footage from anything else.

You only need to record needle drops if paying per needle drop and runnign time they are used.

That's not entirely true. For a few features I worked on, when it came time for distribution, we had to notate every single piece of music on the cue sheets, even music we still owned the publishing rights to and had commissioned for the movie.
