    Either Sony Vegas or Adobe Premiere for the PC Vegas is cheaper but most people go for Premiere. Don't disregard Vegas though, because more and more production companies are using it... including the BBC for Top Gear.

    Most semi pro cameras have the ability to set the levels manually... if so there is a level meter and -12db is one of the figures on that scale. The audio should average about that point. In most shoots the sound guy will have a meters on his kit... so the right levels are going to camera.

    In the UK it's traditional for the production to cover equipment insurance, simply because if you hired the gear and the DOP separately then you'd have to. Therefore you're asking the DOP to take a less good deal than a rental company. As DOP's with their own kit normally work out cheaper...
