It all depends on the execution of the shot and the scene. Write it, film it, put it together, and then decide if it works. If it doesn't, cut it. That simple. ;)

There's no easy answer to that. There are basic "rules" to writing a screenplay. Syd Field has a great book on it, so does Robert McKee. If you read either one of them, they'll give you excellent information on the structure and makeup of a properly written screenplay. That being said, I think...

Is it a short or a feature length? If it's a short, it's best to get right to the action. Maybe a plate showing the production company name, but I wouldn't do a full logo unless it's super short.

I wouldn't re-shoot. I would simply use ADR. It will be a task but it can be done. My last short had completely unusable sound from the shoot, so I had my actors come in and re-do every line. It made a HUGE difference. Here's the finished product. Far from perfect but definitely usable...
